Details
  • Department: Skilled Nursing
  • Schedule: Full-Time, Days & Nights (will include rotating weekends/holidays) & PRN available; 12 hour shifts
  • Facility: Sacred Heart Village
  • Location: Avilla, IN
  • Salary Range: $31.28 - $46.92 per hour

Responsibilities

Provide direct nursing care to patients in the long-term care setting in accordance with established policies, procedures and protocols of the healthcare organization.

  • Implement and monitor patient care plans.
  • Monitor, record and communicate patient condition as appropriate.
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care.
  • Note and carry out physician and nursing orders.
  • Assure resident safety by identifying safety hazards and emergency situations and initiate corrective action as appropriate.
  • Assess and coordinate patient's discharge planning needs with members of the healthcare team.
Requirements

Licensure / Certification / Registration:

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Registered Nurse obtained prior to hire date or job transfer date required.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
