A leading company is seeking a Care Coordinator to manage care coordination for aging members in the community. This remote position requires a licensed nurse with experience in community health and case management. The role involves developing care plans and collaborating with various health service stakeholders.
The Care Coordinator will oversee care coordination and complex case management functions for PathWays to Aging members who live in the community and are not receiving Home and Community Based Services (HCBS) or designated as Nursing Facility Level of Care (NFLOC).
The Care Coordinator must, at a minimum, be a licensed nurse or similar medical professional with extensive experience in providing care coordination to members aged 60 and older. This position requires local travel.
This individual will work directly under the Health Services Directors to maintain the care coordination program and will be responsible for overseeing care coordination teams, developing and implementing care plans.
The Care Coordinator will direct the activities of care coordinators, including physical and behavioral health care services. They will collaborate with the Health Services Director, Service Coordinator Administrator, Medical Director, Provider and Member Services Managers, and with State staff as necessary to communicate with providers and members.
The Care Coordinator will also provide input at State-level meetings as requested.
