Job summary

LeadingAge Virginia seeks a skilled registered nurse to provide comprehensive patient management, care coordination, and discharge planning across inpatient and outpatient settings. The role supports Veterans and families in navigating care options and resources through a collaborative care team.

The Nurse Navigator will address clinical and psychosocial needs, demonstrate leadership in care delivery, and contribute to quality improvement while adhering to VA standards and policies.

Qualifications

• English proficiency required for direct patient care (as per 38 U.S.C. 7403(f)).
• Graduate of a nursing program approved by ACEN or CCNE.
• Current, full, active, unrestricted RN license in a US state or DC.
• Bridge program participants may become registered as a nurse with a state board prior to completion.

Responsibilities

• Provides patient management, care coordination, and discharge planning in inpatient and outpatient settings.
• Assists Veterans, families, and caregivers in receiving appropriate options and services.
• Care Navigator addresses psychosocial and nursing needs with the interdisciplinary team.
• Executes leadership and contributes to process improvement within the care team.
• Adheres to established policies, standards of care, and participates in program development.
