The VA NJ Health Care System seeks a motivated, experienced RN for the Referral Coordination Team (RCT). The RN reviews triages specialty care consults, evaluates clinical documentation, applies clinical guidelines and SLAs, and collaborates with interdisciplinary teams to ensure timely access to specialty care for Veterans.

Strong clinical judgment, communication, organization, adaptability and critical thinking are required to excel in this role within the Veterans Health Administration.

Qualifications

• English language proficiency required.
• RN with state licensure in good standing.
• Graduate of an accredited nursing program (ACEN/CCNE).
• Licensure path or bridge program details as per VA standards.

Responsabilités

• Triage and direct referrals for timely specialty care.
• Review clinical documentation and apply guidelines and SLAs.
• Collaborate with interdisciplinary teams to coordinate care.
• Serve as liaison between VA providers and specialty services.
• Ensure accurate and efficient referral processing and scheduling.
