RN-Recovery Care Coord (REMOTE-NOCTURNAL)

Schedule 6p-4a ($/10 hour shifts weekly, with occasional weekend flexibility)

Responsibilities:

  • Provide after-hours triage support and act as a patient advocate throughout the care process.
  • Coordinate with providers and patients when condition changes necessitate escalation.
  • Monitor and review remote alerts from patients' homes during after-hours.
  • Follow escalation protocols, communicate with on-call clinicians, and update patient treatment plans accordingly.
  • Generate and present operational, clinical, and quality reports.
  • Manage workflows related to patient condition changes and clinical needs.
  • Collaborate with directors and managers to improve documentation and care management.
  • Build and maintain professional relationships with physicians in various markets.
  • Address emergencies via telephone and document interactions in relevant systems.
  • Perform additional duties as assigned.

Qualifications:

  • Associate Degree in Nursing (ADN) or higher.
  • Active RN license in good standing, with willingness to obtain multi-state licenses at Contessa's expense.
  • 3-5 years of acute nursing experience and at least 2 years in telephonic triage.
  • Knowledge of clinical standards and outcome measurement.
