RN-Recovery Care Coord (REMOTE)

Schedule: Fri, Sat, Sun, Mon (7a-5:30p CST)

Responsibilities

  • Facilitates communication and coordination between all members of the care team.
  • Assists patients throughout the care model by acting as a patient advocate and navigator.
  • Connects with providers and patients regularly via telehealth, receiving updates on patient activity including vital signs and assessments.
  • Coordinates referrals and resources to support patient and caregiver continuation of care in outpatient settings.
  • Tracks and prioritizes high-risk patients, ensuring access to appropriate services.
  • Maintains documentation on interactions, visits, interventions, issues, and care plans.
  • Generates and presents operational, clinical, and quality reports, including market updates.
  • Manages clinical and operational workflows.
  • Provides excellent service to internal and external customers at all times.
  • Evaluates and documents healthcare goals, communicating environmental barriers to providers.
  • Communicates discharge information to relevant clinical team members.
  • Identifies system trends and challenges, proposing solutions to improve quality and satisfaction.
  • Builds collaborative relationships with physicians, medical directors, clinicians, and the community for effective care management.
  • Monitors patient care and alerts providers to concerns.

Qualifications

  • Associate of Science in Nursing.
  • Current RN license in the state of employment.
  • Three to five years of acute RN experience.

Preferred

  • Bachelor of Science in Nursing.
  • Care Coordination and Transition Management (C.C.C.T.M) certification.
