Job summary

Healthmap Solutions seeks a Registered Nurse Quality Practice Advisor to coordinate performance improvement activities and partner with the Care Navigation team to enhance the Kidney Health Management program. Requires an active RN license, BLS certification within 30 days, and 3+ years in healthcare or managed care, with strong communication and MS Office skills.

Qualifications

• Bachelor's degree required.
• Active, unrestricted RN license required.
• BLS certification required for all field roles (within 30 days of hire).
• 3 years of experience in a health care or managed care setting.
• 3 years of experience in claims or gap closure campaigns preferred.
• 3+ years of progressive experience in healthcare services, clinical operations, quality, or care management.

Responsibilities

• Coordinate performance improvement activities with Care Navigation to engage, educate, and improve health outcomes.
• Identify opportunities to improve health outcomes based on provider data.
• Educate providers on best practices and close identified care gaps.
• Support provider office workflow improvements and care coordination initiatives.
• Maintain thorough documentation of provider meetings and NCQA compliance.
