Job summary

Community Health Centers of the Rutland Region seeks an experienced Clinical Practice Leadership RN to guide clinical practice standards, quality improvement initiatives, and regulatory readiness. You will oversee vaccine management, infection prevention, and safe, patient-centered care across CHCRR facilities.

In this role you will collaborate with Practice Managers, RN Supervisors, and medical leadership to implement program changes, strengthen accountability, and advance Quadruple Aim goals

Qualifications

• Graduate of an accredited RN program.
• Minimum 5 years clinical nursing experience.
• Experience in primary care, ambulatory care, FQHC, or community health setting preferred.
• Experience with HRSA, PCMH, quality improvement, and regulatory compliance preferred.
• Strong knowledge of primary care operations.
• Strong communication and leadership skills.

Responsibilities

• Lead clinical practice standards across Community Health Centers of the Rutland Region.
• Drive quality improvement initiatives and regulatory readiness.
• Oversee vaccine program and infection prevention.
• Support site-level accountability and operational consistency.
• Collaborate with managers, supervisors, providers, and educators.
• Facilitate implementation of clinical programs and standard workflows.
• Develop corrective action plans based on audits and trends.
• Promote patient-centered, safe, and efficient care delivery.
