Job summary

Community Health Centers of the Rutland Region (CHCRR) seeks a Clinical Practice & RN Quality Manager. This RN will lead clinical practice standards, drive quality improvement, ensure regulatory readiness, oversee vaccine programs, and manage infection prevention across Rutland region sites.

Qualifications

  • Vermont RN license in good standing.
  • Graduate of an accredited RN program.
  • Minimum 5 years of clinical nursing experience.
  • Experience in primary/ambulatory care preferred.
  • Experience with HRSA, PCMH, quality improvement and regulatory compliance preferred.

ABOUT THE ROLE:

The Clinical Practice and RN Quality Manager is a Registered Nurse responsible for leading clinical practice standards, quality improvement initiatives, regulatory readiness, vaccine program oversight, infection prevention, and operational consistency across Community Health Centers of the Rutland Region. Working under the direction of the Director of Clinical Operations, this position serves as the organization’s clinical practice and quality resource, ensuring safe, compliant, standardized, and patient-centered clinical operations.

This role collaborates closely with Practice Managers, RN Supervisors, providers, the Clinical Nurse Educator, and the Clinical Practice & Education Support Specialist to support consistent implementation of clinical standards, strengthen site-level accountability, and advance CHCRR’s strategic goals, Quadruple Aim initiatives, and team-based care model.

FUNCTIONS OF THE POSITION:

Clinical Practice Leadership
  • Develop, implement, and maintain organization-wide clinical standards and best practices.
  • Ensure clinical policies and procedures align with regulatory requirements and evidence-based practice.
  • Evaluate clinical processes and recommend operational improvements
  • Serve as the primary resource for clinical operations, scope-of-practice, and workflow questions.
  • Lead implementation of new clinical programs and operational initiatives.
  • Evaluate staffing workflows and support clinical efficiency initiatives.
Quality Management & Performance Improvement
  • Develop and oversee clinical practice audit processes to monitor compliance with organizational standards.
  • Conduct clinical quality reviews and identify opportunities for clinical practice and workflow improvement based on audit findings, clinical trends, and recommendations from clinical committees and medical leadership.
  • Analyze audit findings and partner with site leadership to develop corrective action plans.
  • Collaborate with Quality, Population Health, and Provider Leadership to support improvement in clinical outcomes and patient experience.
  • Lead clinical operations improvement projects that reduce variation and support safe, efficient care delivery
Regulatory Compliance & Accreditation Readiness
  • Support clinical components of Patient-Centered Medical Home (PCMH) initiatives and other applicable regulatory and accreditation requirements, such as HRSA and FTCA
  • Coordinate clinical aspects of regulatory reviews, surveys, and audits in collaboration with appropriate organizational leaders.
  • Develop corrective action plans when compliance gaps are identified.
  • Maintain oversight of policies affecting clinical operations and patient safety.
Serve as the organizational owner of the vaccine management program, including:
  • Oversight of VFC participation and compliance.
  • Develop and maintain organization-wide vaccine policies, workflows, and storage and handling standards.
  • Coordination with RN Supervisors and site vaccine coordinators to ensure consistent site-level vaccine operations including vaccine inventory standards, temperature excursion response, and regulatory documentation.
  • Support of vaccine-related audit readiness, corrective action planning, and process improvement.
  • Lead organization-wide infection prevention initiatives in collaboration with site leadership.
  • Maintain and support implementation of infection control policies and procedures.
  • Review clinical incidents and patient safety events to identify trends and opportunities for improvement.
  • Develop risk-reduction strategies and corrective action plans when clinical practice concerns are identified.
  • Collaborate with safety committees and organizational leadership to support a culture of safety.
Clinical Equipment & Infrastructure
  • Evaluate and standardize clinical equipment across practice sites.
  • Monitor compliance with equipment maintenance, calibration, and safety requirements.
  • Support evaluation of new clinical equipment and technology to promote consistency, safety, and operational efficiency across practice locations.
  • Partner with clinical and operational leaders regarding equipment needs, purchases, and replacement planning.
  • Develop and maintain equipment standardization and lifecycle management processes.
OTHER DUTIES:
  • Attends meetings as required.
  • Works collaboratively as a member of the organizational team and follows the Community Health Code of Conduct.
  • Performs additional responsibilities as assigned that are consistent with the scope and purpose of the position
SKILLS REQUIRED FOR SUCCESS:
  • Maintains current Vermont State RN License in good standing
  • Graduate of an accredited RN school of nursing program
  • Minimum 5 years clinical nursing experience.
  • Experience in primary care, ambulatory care, FQHC, or community health setting preferred.
  • Experience with HRSA, PCMH, quality improvement, and regulatory compliance preferred
  • Graduate of an accredited RN school of nursing program
  • Strong knowledge of primary care operations.
  • Strong communication and leadership skills.
  • Ability to lead organizational change initiatives.
  • Strong analytical and problem-solving abilities
