Personal Touch Home Care in Brooklyn, NY is seeking an RN Quality Review Manager to coordinate and supervise clinical personnel and ensure high-quality home care services. Qualifications

• RN licensed in New York with BSN preferred.
• Minimum 2 years of home health care experience.
• At least 1 year of management or supervisory experience in a health care setting, preferably home care.
• Excellent observation and communication skills; English language required.
• Strong written communication.

Responsibilities

• Reviews referrals to determine home care needs.
• Reviews case, provides guidance to clinicians to improve care.
• Establishes clinical diagnoses with ICD-10 coding.
• Assists in establishing POC with med review and goals.
• Oversees patient care services and personnel.
• Prospective review of OASIS assessments (Start of Care, Resumption of Care, Recertification, Transfers, Discharge).
• Directs case conference meetings and coordinates care.
• Conducts quarterly record reviews and communicates findings.
• Assists in orientation and education programs.
• Performs direct patient care duties as appropriate.
• Performs other duties as assigned.
