• Manage complex patient recovery journeys by monitoring physiological changes and adapting care plans for safety during the transition toward healing.
• Advocate for patient needs by coordinating between physicians, therapists, and specialists for a unified approach to treatment.
• Execute clinical orders with precision and translate medical instructions into compassionate hands-on care for high-acuity patients.
• Assess discharge readiness and collaborate with the healthcare team to provide patients and families with tools for a successful recovery.
