Rochester Regional Health's DeMay Living Center in Newark, NY is seeking a Program Manager- Registered Nurse to lead clinical practice and patient care excellence within a long-term care setting.

The role oversees referrals, psychosocial assessments, discharge planning, and collaboration with families, staff, and physicians. It supports leadership development and quality improvement initiatives in a team-based environment.

Qualificações

• Associate's degree in Nursing (RN) is required.
• Current NYS Registered Nurse license and BLS certification or ability to obtain within 2 weeks of hire.
• Minimum 1 year experience on a nursing unit.

Responsabilidades

• Receives referrals for high-risk neurobehavioral cases from admissions; ensures correct channeling and documentation.
• Performs psychosocial assessments through patient/family interviews and records in charts.
• Implements discharge/continued care plans in compliance with regulations.
• Involves patient/family in treatment planning with documented chart notes and signatures.
• Executes discharge/continued care plans mutually agreed with patient/family.
• Monitors operations for licensure, accreditation, and regulatory compliance.
