RN-Program Coordinator/Part Time At SSM Health Summary

The RN-Program Coordinator is responsible for the coordination of the structural heart program, working closely with various departments to provide patient-centered care. This role involves managing patient data, coordinating educational outreach, and ensuring timely communication between patients and healthcare providers. The RN will also facilitate assessments, develop treatment plans, and support program implementation while maintaining high standards of patient care.

Job Highlights
  • The RN in this position is responsible for care coordination of the structural heart program, working with the current structural heart coordinator. Work schedule involves two days one week and three days the next, Monday through Friday, with potential after-hours or weekend procedures.
Responsibilities
  • Drive a quality, patient-focused program, managing implementation, patient/physician notification, education, and outreach.
  • Collect and analyze patient data from various sources.
  • Develop communication pathways for timely data sharing.
  • Assist with patient referrals and follow-up to ensure appropriateness and satisfaction.
  • Maintain and review data in valve tracking database.
  • Develop and revise educational materials.
  • Provide education to care team and patients.
  • Coordinate testing, scheduling, and procedures.
  • Participate in weekly conferences and establish relationships with surgical and interventional teams.
  • Adapt to evolving technologies such as TAVR, MitraClip, WATCHMAN.
Experience/Attributes
  • BSN preferred.
  • Knowledge of cardiovascular health and diagnostics.
  • Diplomatic, proactive, and excellent communication skills.
  • Ability to manage multiple priorities and educate diverse groups.
  • Strong problem-solving and multitasking skills.
Job Summary

Coordinate activities and workflow for the clinical program, ensuring quality and compliance.

Primary Responsibilities
  • Develop, implement, and evaluate program services.
  • Coordinate patient care and maintain documentation for regulatory compliance.
  • Assist in staff training and collaborate with providers to improve outcomes.
  • Participate in injury prevention and other initiatives.
  • Perform duties safely and effectively.
Education
  • Bachelor's degree in Nursing or equivalent experience.
Experience
  • At least three years of RN experience.
Physical Requirements
  • Frequent lifting, sitting, standing, walking, and use of vision and hearing.
  • Occasional bending, lifting over 25 lbs, and climbing.
Licenses and Certifications
  • BLS certification within 90 days of employment.
  • RN license in Illinois, Missouri, Oklahoma, Wisconsin, or a Compact State, as applicable.
Work Details
  • Day shift, part-time (20 hours/week).
  • Location: WI-St. Agnes Hospital.
