Avamere Rehabilitation at Ridgemont in Port Orchard, WA is seeking a Registered Nurse (RN) to join our PRN schedule across all shifts. The role involves comprehensive patient care, medication administration, and coordination with a multidisciplinary team to support resident well-being.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, active license to practice as an RN in WA; BSN preferred.
• Current CPR/BLS certification required.
• Experience in long-term care or geriatric rehab preferred.
• Experience with electronic medical records documentation.

Responsibilities

• Patient assessment and care planning: Assess residents' physical, cognitive, and emotional needs.
• Medication administration and monitoring: Administer medications and monitor responses.
• Vital signs and health monitoring: Document vital signs and report changes.
• Wound care and specialized care: Coordinate with wound care specialists to support healing.
• Supervision and training: Supervise LPNs and CNAs; provide orientation.
• Communication and documentation: Maintain accurate resident care records and coordinate with the team.
• Quality improvement: Participate in quality initiatives to improve care.
