Summary of Job Duties: The RN Care Manager manages the continuum of care from admission through discharge, adhering to RN scope of practice, state requirements, CMS Conditions of Participation, EMTALA, Patient Bill of Rights, AB 1203, and other regulations. The role involves partnering with a multidisciplinary team, utilizing evidence-based practices, and ensuring compliance with UAMS’s mission and values.

Qualifications:

Minimum Qualifications:

  • Baccalaureate Degree in Nursing, ADN, or Diploma RN Degree
  • Three (3) years RN experience with current BLS certification
  • Valid Arkansas Registered Nursing License

Preferred Qualifications:

  • National Nursing Certification
  • BSN preferred; Certified Case Manager (CCM) or American Case Management (ACM) certification

Additional Responsibilities/Competencies:

  • Excellent communication skills
  • Basic computer skills
  • Strong work ethics and accountability

Additional Information:

  • Initiates referrals to interdisciplinary teams.
  • Establishes working diagnoses and collaborates on discharge planning.
  • Documents communications and concerns regarding care coordination.
  • Communicates with physicians about care levels and admission status.
  • Proactively manages discharge timing, documenting avoidable and denied days.
  • Refers psychosocial or non-medical discharge needs to Social Work.
  • Assesses and revises discharge plans as needed.
  • Identifies cases for Ethics Committee review.

Education/EBP/Research /QI:

  • Participates in quality improvement, evidence-based practice, and research.
  • Supports current evidence integration into nursing practice.
  • Provides patient and family education.

Communication/Collaboration:

  • Implements patient-centered care approaches.
  • Collaborates effectively with the multidisciplinary team.
  • Documents care appropriately in medical records.
  • Reports variances in care/system processes.

Leadership:

  • Acts as a resource to colleagues.
  • Supports professional development of staff.
  • Participates in recruitment and standards development.

Performs other duties as assigned.
