Job summary

Home Health of Western Colorado is looking for a Registered Nurse (RN) to join their team in Montrose, CO. This role involves providing comprehensive care management and coordination for clients while working in a supportive and dedicated environment.

The ideal candidate will have at least two years of nursing experience and a current RN license in Colorado. The position offers competitive pay ranging from $40.07 to $48.10 per hour based on experience and includes a variety of benefits and opportunities for career growth.

What You Bring (Requirements)

• Graduate of an accredited school of professional nursing program; Baccalaureate degree preferred.
• Current license as an RN in the state of Colorado.
• Minimum of two (2) years of nursing experience, with a combination of long‑term care, dementia care or acute care; home care experience preferred.
• CPR certified.

What You’ll Be Responsible For

• The Registered Nurse (RN) provides case management, coordination, supervision and implementation of professional and supportive services to clients using nursing theory and process in accordance with the Nurse Practice Act.
• Reviews client referral information and responds to requests and inquiries as appropriate.
• Performs client assessments based on agency standards of practice to ensure effective and appropriate home care services.
• Directs, plans and initiates appropriate action independently and responsively in home‑care situations.
• Performs job duties in accordance with agency policies, procedures, and professional and community standards.
• Performs other job duties as assigned by Director of Clinical Services.

Home Health of Western Colorado is a Medicare/Medicaid‑certified home health care agency in Montrose, CO which includes an experienced team of nurses, rehab therapists and home health aides dedicated to providing individualized health care needs with compassion and empathy. We are trained in caring for older adults, providing in‑home health care services in the Delta, Montrose and Ouray counties and Montrose, CO.
