Job summary

Family Care Health Centers in Missouri seeks a Registered Nurse to provide comprehensive nursing services across primary care settings. You will document in the EMR, support care transitions, and apply disease management education, with emphasis on patient-centered care and population health outcomes.

Qualifications

• Graduate of an accredited nursing program; MO RN licensure required.
• Two+ years of nursing experience in a setting with care coordination.
• BSN preferred; OB experience beneficial.

Responsibilities

• Provide comprehensive nursing services, including assessment and care coordination.
• Perform triage, education, referrals, and care planning.
• Document care in the EMR and coordinate with multidisciplinary team.
• Administer injections, vital signs, and routine RN duties.
• Participate in huddles and care transitions including post-discharge follow-up.
