Southcoast Health System, Inc. is seeking an experienced RN-Practice Navigator – Oncology to join our Cancer Center team in Fairhaven, MA.
This role focuses on coordinating comprehensive cancer care, guiding patients from diagnosis through survivorship, and collaborating with a multidisciplinary team to optimize outcomes. The ideal candidate will have a strong oncology background, BSN or higher, and OCN certification or willingness to obtain within one year.

Hours: 40hrs
Shift: Day shift; 8:00am - 4:30pm
Location: Cancer Center – Fairhaven, MA

Under direction of Nurse Manager or designee, provide consistent care coordination across the continuum, assessing the physical, psychological and social needs of the patient. Serve as ongoing resource for patients during the initial diagnosis and throughout treatment and survivorship. Organize and facilitate clinic visit needs. Assist patients and families by providing education, support and information regarding available resources for cancer patients within the Cancer Center and the community. Provide educational materials and referrals to appropriate support groups and services. Support patient and families to ensure compliance with care.
