Job summary

Southcoast Health System, Inc. is seeking an experienced RN-Practice Navigator – Oncology to join our Cancer Center team in Fairhaven, MA.

This role focuses on coordinating comprehensive cancer care, guiding patients from diagnosis through survivorship, and collaborating with a multidisciplinary team to optimize outcomes. The ideal candidate will have a strong oncology background, BSN or higher, and OCN certification or willingness to obtain within one year.

Qualifications

• Strong oncology background required.
• Certification like OCN or willingness to obtain within 1 year of hire.
• BSN or equivalent experience required.
• Oncology Nurse Certification preferred; must obtain within 18 months of employment.
• Massachusetts RN licensure and CPR preferred.

Responsibilities

• Provide consistent care coordination across the continuum for cancer patients.
• Serve as ongoing resource for patients from diagnosis through survivorship.
• Organize and facilitate clinic visit needs and education.
• Refer patients to support groups and services within the Cancer Center and community.
• Support families to ensure treatment plan adherence.
