• Select and onboard staff; supervise PACCs and training.
• Collaborate with hospital teams to manage discharge planning.
• Directly supervise department employees and address employee relations.
• Maintain knowledge of Medicare/insurance guidelines and hospital policies.
• Minimize patient risk and promote patient well‑being.
• Coordinate care conferences and ensure issue resolution.
