Job summary

Redwoods Post Acute, located in the heart of Willow Glen, San Jose, CA, seeks a Licensed Vocational Nurse (LVN) to join our skilled nursing facility team. You will administer medications, assist with daily living activities, and collaborate with RNs to implement individualized care plans.

Freshly licensed LVNs are welcome; experienced LVNs are preferred. Active BLS and CPR certifications are required, with a passion for geriatric care and patient education.

Qualifications

• LVN license in California is required/active.
• BLS and CPR certifications are required.
• Experience in geriatric/long-term care is a plus.

Responsibilities

• Administer medications and monitor patient reactions.
• Assist with activities of daily living (bathing, grooming, feeding).
• Maintain accurate patient records and documentation.
• Collaborate with RNs and the care team to implement care plans.
• Perform routine health assessments and educate patients/families.
