Job description

Health Advocates Network is seeking a RN Population Health (Aviation) to work at a facility in Oak Harbor, WA. This is a registry position with our company.

Pay Rate:

$40 / hour

Position Details:
  • W2 employment status
Responsibilities:
  1. Serve as a Nurse for Population Health Improvement, managing health metrics and outcomes for the primary pediatric care population.
  2. Coordinate Population Health for enrolled beneficiaries, acting as the Clinical Practice Guideline Coordinator within the Military Health System.
  3. Utilize Population Health Portal (MHSPHP) and related web applications to analyze outcomes data and determine program needs.
  4. Implement clinical practice guidelines to support prevention and disease management initiatives, collaborating with healthcare providers, staff, and patients.
  5. Standardize Disease Management across the patient care continuum, monitoring adherence to clinical guidelines and promoting evidence-based practices.
  6. Act as a link within the PCMH practice to connect healthcare providers, primary care team members, and Nurse Case Managers to identify beneficiaries' healthcare needs.
  7. Work closely with Nurse Case Managers and the primary care team to promote a whole-person approach during patient encounters.
  8. Engage proactively with care teams and patients to integrate population health activities into patient care, providing data support to stakeholders.
  9. Apply the six core principles of population health management—identification and assessment, demand forecasting, demand management, capacity management, evidence-based care and prevention, and program evaluation—to deliver population-based interventions.
