Health Advocates Network is seeking a RN Population Health (Aviation) to work at a facility in Oak Harbor, WA. This is a registry position with our company.

Pay Rate: $40 / hour

W2

Responsibilities
  1. Serves as Nurse for Population Health Improvement. Manages the health metrics and outcomes of the primary pediatric care population.
  2. Coordinates Population Health for enrolled beneficiaries and manages the Military Health System Population Health Portal (MHSPHP) and related web applications to determine program needs.
  3. Implements clinical practice guidelines, supporting AMEDD prevention and disease management initiatives through collaboration with healthcare providers, staff, and patients.
  4. Standardizes Disease Management across the patient care continuum, monitoring adherence to clinical guidelines and integrating evidence-based practices.
  5. Acts as a link within the PCMH practice between providers, care team members, and Nurse Case Managers to identify beneficiaries' healthcare needs.
  6. Works with Nurse Case Managers and primary care teams to promote a whole-person approach within each encounter.
  7. Engages proactively with care teams and patients to incorporate population health activities into patient encounters and provides data support for population health management.
  8. Utilizes core principles of population health management to deliver targeted health interventions.
