Job summary

Hospice of the Valley is seeking a Phone RN to triage calls from patients, families and caregivers, delivering medical and emotional interventions for symptom management, coordinating visit nurses on shift, and serving as a member of the interdisciplinary care team from our Phoenix office.

Based out of our Central Clinical office in Phoenix, this role emphasizes telephone-based scope of practice, documentation, teamwork and a commitment to compassionate, patient-centered care.

Qualifications

• Graduate of accredited school of nursing.
• Minimum one year of recent experience in medical, surgical or critical care as a professional nurse.
• Strong clinical assessment skills.
• Excellent communication and interpersonal skills.
• Solution-driven, creative problem solving skills.
• Minimum two years of recent experience in medical, surgical or critical care nursing.

Position Profile

The Phone RN triages phone calls from patients, families and caregivers applying medical and emotional interventions for symptom management; informs team members of changes and coordinates the work of visit nurses on assigned shift. Serves as member of an interdisciplinary care team. Scope of practice in this role is provided solely via telephone consultation. This position is based out of our Central Clinical office located in Phoenix.
