Job summary

Eagle Home Care, a LHCSA, is seeking motivated and compassionate Registered Nurses to perform home health assessments and provide high-quality patient care. This per diem, part-time role offers flexible scheduling and competitive compensation.

Responsibilities include conducting health assessments in patients' homes, training aides on care plans and infection control, and maintaining timely communication with the clinical department. NYS RN license and ability to travel are required.

Qualifications

• Graduate of approved nursing program.
• Current NYS RN license.
• 1 year of home health nursing experience preferred.
• Able to function independently with minimum supervision.
• Demonstrates strong critical thinking skills.
• Ability to travel to patient’s homes.

  Duties:




• Seeking a professional Registered Nurse to perform health assessments for patients in their home to assess a patient’s health care status and needs




• Monitor and train Aide staff on patient plan of care and infection control




• Maintain close communication with the Director of Patient Services in the office, responding to phone calls and emails in a timely manner.




• Reporting all issues and concerns to the clinical department immediately.




• Completes documentation accurately and submits timely to agency




• Provides the highest quality care in accordance with DOH regulations
