Job summary

Live Well Homecare in Dallas, TX seeks a Patient Placement and Transfer Coordinator RN to orchestrate patient moves within the hospital system, working closely with physicians, nurses, and departments to ensure timely and compliant placements.

You will handle daily placement calls, adhere to COBRA and EMTALA guidelines, match patient needs to available resources, and provide exceptional service in a fast-paced healthcare environment.

Qualifications

• Graduate of an accredited nursing program; BSN preferred or ADN with equivalent experience.
• Minimum of 3 years healthcare experience, including at least 2 years in acute hospital settings.
• Must hold a current RN license in Texas.
• Physical ability to lift, push, and pull weighted objects up to 50 lbs, with assistance.
• Strong communication, organizational, and problem-solving skills.

Responsibilities

• Facilitate patient transfers within the hospital system, coordinating with physicians, staff, and departments to ensure timely and compliant placements.
• Manage daily calls for patient placement, ensuring adherence to protocols and regulations such as COBRA and EMTALA.
• Match patient needs with system resources to optimize care, provide follow-up communication, and report workflow trends for continuous improvement.
• Deliver exceptional customer service and maintain high standards of professionalism in a fast-paced environment.
