Job description

We Are Hiring
  • Department: Executive Health
  • Schedule: Day Shift: Monday-Friday 8am-4:30pm (No nights, weekends or major holidays)
  • Hospital: Ascension Medical Group, clinic
  • Location: 8333 Naab Rd. Indianapolis, IN

BSN, seasoned healthcare professional, and preventive healthcare experience preferred.

What You Will Do

Serve as a patient navigator throughout the continuum of care.

  • Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other stakeholders.
  • Coordinate treatment efforts and promote quality improvement and educational initiatives.
  • Compile data, track outcomes, and recommend process improvements.
  • Provide referrals and support for follow-up care for uninsured or underinsured patients.
What You Will Need

Licensure / Certification / Registration:

  • Registered Nurse credential from the Indiana Board of Nursing is required prior to hire or transfer.
  • BLS Provider certification preferred, with American Heart Association or American Red Cross accepted.

Education:

  • Diploma from an accredited nursing school or college, or professional licensure at the time of hire.
