Job summary

Baylor Scott & White Health in Fort Worth is seeking a Patient Navigator RN to coordinate care for transplant patients within a multidisciplinary team. You will assess patient needs, plan and implement care, and advocate for patients navigating complex pathways.

The role requires five years of RN experience, region-wide travel across Texas, and strong communication with physicians, clinics, and families. BSN and Epic EMR experience are preferred.

Qualifications

• Willingness to travel regionally across Texas for outreach (no on-call).
• Bachelor's degree or higher required; RN license active.

Responsibilities

• Coordinate care for transplant patients with the medical team.
• Plan, implement, and evaluate healthcare with the team.
• Advocate for patients and serve as educator and coordinator.
• Schedule outpatient tests and maintain patient communication.
• Maintain nursing records and update the Navigator database.
• Participate in community outreach and case conferences.

PATIENT NAVIGATOR

Location: Baylor All Saints – Fort Worth

Specialty: Transplant

Shift: FT Days, Monday-Friday, 8:00am-4:30pm; flexibility is needed for community events after work hours.

Requirements: 5 years of recent RN experience; Willingness and ability to travel regionally for clinical outreach, including routine local travel and occasional travel to outreach communities throughout Texas to build collaborative partnerships with patients, providers, and clinical teams (no on-call requirements).

Preferred Experience & Highly Preferred Skills

• Clinical background or specialized clinical knowledge in organ transplantation, hepatology, gastroenterology (GI), or oncology.
• Experience delivering clinical program education (providing education to transplant and services offered, depending on audience), care pathway guidelines, or educational presentations to physicians, clinical staff, and healthcare professionals.
• Prior experience managing regional healthcare relationships, coordinating clinical transitions across external practices, or serving as a key point of contact for external medical teams.
• Experience collecting, tracking, and reviewing clinical patient referral data or program metrics.(via Epic)
• Proficiency with Electronic Medical Records (preferably Epic).
• Bilingual (Spanish speaking) is preferred, but not required.

Job Summary

As a Patient Navigator RN, ensure smooth patient care coordination. Assess patient needs and help plan, implement, and evaluate healthcare with the medical team. Work autonomously and with the team. Also serve as advocate, teacher, and coordinator for patients.

Essential Functions Of The Role

• Care for patients in all aspects by evaluating their health based on doctor's orders and creating a healthcare plan.
• Schedule outpatient tests for diagnosis, stay in touch with the healthcare team.
• Visit doctors often, support patient-office communication.
• Collaborate with healthcare professionals for optimal patient care.
• Conduct follow-ups every 3 months and 1 year.
• Maintain up-to-the-minute and comprehensive nursing records, also updating patient details in the Patient Navigator database.
• Attend case conferences to expand your knowledge. Participate in community service events.
• Work together on improvement projects to enhance service for patients, families, and the healthcare system.
• Welcome patients and families, offer facility details, directions, and general information.
• Assist in guiding patients and families through the hospital, especially to hard-to-find places.

• A Bachelor's degree.
• Registered Licensed Nurse.
• You have, or are willing to obtain within your first 30 days, a Basic Life Support (BLS)
