Qualifications

Responsibilities

• Coordinate patient movement with referring agencies, hospitals, clinics, and EDs to ensure safe transfers.
• Provide 24/7 coverage for transfer needs in collaboration with internal and external clients.
• Navigate complex medical systems with a patient-centered approach, prioritizing acuity and safety.
• Support collaboration with clinical teams to optimize patient flow within the Duke Health System.
