Job summary

Qualifications

• Specialized Diploma graduate of an accredited nursing program.
• Knowledge of patient- and family-centered care.
• Cultural awareness and ability to connect with diverse populations.
• Effective communication and collaboration with patients, families, and teams.
• Knowledge of discharge planning and post-discharge needs.
• BLS certification; PA RN license.
• Active RN license in state of practice.

Responsibilities

• Collaborate with patients and families to identify learning needs and provide education.
• Use information systems to communicate, monitor care outcomes and support improvement.
• Work within inter-professional teams with open communication and shared decision-making.
• Provide safe, compassionate, coordinated care respecting patient preferences and values.
• Contribute to safety and quality committees and participate in ongoing professional development.
• Act as a preceptor and participate in continuous staff education.
