Job summary

Live Well Homecare Agency, LLC is seeking a Registered Nurse to deliver high-quality patient care within a patient care team. The role emphasizes assessment, planning, implementation, and evaluation of patient needs, while ensuring safety, confidentiality, and satisfaction across the care continuum.

You will coordinate care, advocate for patients and families, participate in performance improvement, and support ongoing professional development and leadership growth within a supportive governance

Qualifications

• Associates or Bachelor’s Degree in Nursing (preferred).
• Current RN licensure (single state or multi-state compact) and BLS certification required.
• Experience with direct patient care preferred.

Responsibilities

• Monitor and evaluate patient conditions, assist with care planning, and deliver safe, effective care aligned with policies.
• Coordinate patient care and prioritize daily workflows to optimize patient outcomes and experience.
• Act as an advocate by addressing patient concerns, providing education, and communicating with families and staff.
• Participate in performance improvement initiatives and support organizational goals for quality and satisfaction.
• Maintain professional licenses, certifications, and pursue ongoing development and leadership growth.
• Mentor and support peers, promote professionalism, and uphold shared governance principles.
