Job summary

LiveWell Homecare Agency, LLC is seeking a Registered Nurse to deliver high-quality nursing care in collaboration with the patient care team. The RN will assess, plan, implement, and evaluate patient needs while serving as a liaison between patients, families, and healthcare providers to ensure patient-centered care and safety.

You will participate in performance improvement initiatives, maintain professional competence, and support shared governance activities.

Qualifications

• Associates or Bachelor’s Degree in Nursing preferred.
• Current RN license (single state or multi-state compact).
• Basic Life Support (BLS) certification required.
• Strong communication, critical thinking, and teamwork skills.

Responsibilities

• Monitor and evaluate patient conditions, assist with care planning, document assessments and interventions.
• Coordinate patient care, advocate for patients and educate families.
• Support performance improvement initiatives and quality outcomes.
• Maintain licenses and pursue ongoing professional development.
