Northwestern Medicine seeks an RN with BSN to embody Relationship Based Care principles under department leadership. The nurse will uphold the State Act, Code of Ethics, and Compliance Program while delivering high-quality patient care across the unit.

Responsabilidades

• Performs a comprehensive assessment on all assigned patients as evidenced by completed documentation.
• Communicates changes in patient's condition promptly.
• Utilizes nursing diagnosis and assessment data to derive outcomes and documents measurable goals on the plan of care; modifies care plan as needed.
• Identifies and prioritizes patient needs and documents.
• Collaborates with patient, family, and health care team in planning patient care; assesses discharge planning needs and makes referrals.
• Evaluates patient response to nursing interventions and updates the plan of care accordingly.
• Assesses discharge planning needs and makes appropriate referrals, including palliative care.
• Provides complete discharge instructions, medication reconciliation, and information about patient portal resources to patient/family.
• Supports education, ethics and diversity considerations for patients and families; uses teach-back for medication education; distributes Krames patient education resources; applies culture vision in care planning.
• Implements nursing interventions based on the plan of care and evidence-based practice; utilizes resources like clinical databases; participates in ladder/project or quality initiatives.
• Maintains safe medication administration via bar coding; promotes patient safety; communicates at bedside hand-off; participates in unit safety and quality metrics.
