Job summary

Angels Care Hospice is seeking a Registered Nurse Patient Care Manager (PCM) to coordinate and manage patient care across hospice services. You will review referrals, guide clinical teams, and help set immediate and long-term goals with a focus on pain management and quality care.

The position requires an active RN license in the state, hospice/home care experience, and at least two years in leadership. We value strong observation, communication, and collaborative teamwork.

Qualifications

• Must be a licensed registered nurse in the state.
• BSN degree preferred.
• Experience in hospice and/or home care.
• Minimum 2 years of management or supervisory experience.
• Demonstrates professional standards and judgment.
• Strong observation, nursing judgment and communication skills.
• Understanding hospice philosophy and pain/symptom management.

Responsibilities

• Receives case referrals and reviews patient information to determine hospice care needs.
• Evaluates cases via home visits, conferences, record review and services provided.
• Reviews patient diagnoses, prognosis, medications, procedures, and clinical course.
• Assists clinicians in setting priorities and developing care plans.
• Attends case conferences to coordinate care across disciplines.
• Conducts quarterly record reviews and communicates findings to Clinical Director.
• Assesses health needs of the community and helps develop plans.
• Participates in public relations and community activities.
• Promotes customer service orientation for hospice personnel.
• Participates in quality improvement teams and activities.
