Job summary

Angels Care Hospice in Lubbock, TX is seeking a Registered Nurse Patient Care Manager (PCM) to coordinate and oversee patient care within our hospice program. This role partners with clinicians to develop and implement care plans, ensuring timely, high-quality service.

The ideal candidate is a licensed RN with hospice or home care experience, at least two years of supervisory experience, and a commitment to compassionate, patient-centered care.

Qualifications

  • Licensed RN in the state with current active licensure.
  • BSN degree preferred.
  • Experience in hospice or home care setting.
  • Minimum 2 years of management or supervisory experience.
  • Knowledge of pain and symptom management principles.
  • Strong observation, nursing judgment, and communication skills.
  • Understands hospice philosophy and issues of death/dying.

Responsibilities

  • Receive case referrals and review patient information to determine hospice care needs.
  • Evaluate cases via home visits, conferences, records, and clinician services.
  • Review diagnoses, prognosis, meds, procedures, and clinical course.
  • Assist clinicians in setting goals and developing plans of care.
  • Attend case conferences to coordinate care across the team.
  • Conduct quarterly record reviews and report findings to Clinical Director.
  • Assess community health needs and help develop plans/programs.
  • Participate in community activities and PR.
  • Promote customer service orientation for hospice staff.
  • Participate in quality improvement teams.
  • Perform other duties as delegated by the Executive.

About the position:

The Registered Nurse Patient Care Manager is responsible for ensuring that patient care is coordinated and managed appropriately.

Job Functions:
  • Receives case referrals & reviews available patient information related to case to determine hospice care needs.
  • Evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians.
  • Reviews patient’s medical diagnosis, prognosis, medications, procedures, and clinical course.
  • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plans of care.
  • Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
  • Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director and hospice personnel.
  • Assesses health needs of community and assists in the development of plan(s)/program(s) to meet needs.
  • Participates in public relations and community activities.
  • Promotes customer service orientation for all hospice personnel.
  • Participates actively in quality assessment performance improvement teams and activities.
  • Performs other duties and activities as delegated by the Executive
Qualifications:
  • Registered nurse with current licensure to practice professional nursing in the state.
  • Graduate of accredited school of nursing with a BSN degree preferred.
  • Previous experience in hospice and/or home care setting.
  • 2 years of management or supervisory experience.
  • Complies with accepted professional standards and practice.
  • Has excellent observation, nursing judgment, and communication skills.
  • Understands hospice philosophy and issues of death/dying.
  • Has knowledge of the principles of pain & symptom management.
