HighPoint Hospice, part of the Optum family, is seeking an experienced Patient Care Manager - Hospice to support the Executive Administrator/Director and ensure safe, compliant patient care in home settings. This role emphasizes quality, staff safety, and regulatory adherence while delivering compassionate care.

You will lead clinical teams, promote education, and coordinate services to address patient needs and regulatory requirements, with opportunities for professional growth within the Optum

Qualifications

• Current unrestricted RN licensure in state of practice.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.
• 4+ years full-time experience as an RN or equivalent.
• 1+ years of clinical experience.
• Able to work independently.
• Good communication, writing, and organizational skills.

Responsabilités

• Responsible for compliance with hospice policies and procedures and with local, state, and federal regulatory requirements governing the practice of the assigned team
• Actively promotes and directs assigned team regarding quality of care and safety of patients and staff
• Engages in thorough problem resolution and complaint investigation
• Provides in-services to staff to promote the health and safety of the patient, to familiarize staff with regulatory issues and agency policy and procedures, and to address any identified educational needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Pay is based on several factors including but not limited to local labor markets, education, work experience, certificates etc in addition to your salary we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us you will find a far-reaching choice of benefits and incentives The salary for this role will range from $82,004 to $87,360 annually based on full time employment. We comply with all minimum wage laws as applicable.
