Job summary

Optum is seeking a Home Health Patient Care Manager in the Dallas area to supervise and coordinate clinical services for home health patients. You will lead an interdisciplinary team to ensure high-quality care in accordance with physician plans and state/federal regulations.

Responsibilities include receiving referrals, starting care, coordinating benefits and approvals, and daily review of plans of care. CPR certification and home care experience are preferred.

Qualifications

• Current unrestricted RN licensure in state of practice.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification preferred or ability to obtain within 90 days of hire.
• Home care experience preferred.

Responsibilities

• Provides clinical services within the scope of practice, per plan of care.
• Receives referrals, assigns clinicians, evaluates patient orders, and starts care visits.
• Coordinates determination of patient home health benefits and ongoing insurance approvals.
• Ensures patient needs are assessed and care is individualized and compliant with physician orders.
• Reviews assessments and plans of care daily and consults clinicians as appropriate.
• Works effectively in diverse home situations and with patients of all backgrounds.
