Job summary

Job Juncture seeks a Patient Care Manager to lead the in-field clinical team and oversee delivery of high-quality patient care in the United States, Pennsylvania, Marysville area.

The role requires licensure in the state, extensive supervisory experience in home care, and strong knowledge of regulatory requirements, including Medicare Conditions of Participation. You will supervise staff, coordinate care, and drive quality outcomes while maintaining professionalism and safety standards.

Qualifications

• Must hold a current state license or be reinstatable.
• Two years of management or supervisory experience in home care.
• Knowledge of Medicare Conditions of Participation and regulatory requirements

Responsibilities

• Lead in-field clinical team to ensure patient care delivery at high standards.
• Provide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirements.

Patient Care Manager Qualifications

• Must have a License in the state or an expired license from the state that can be reinstated would work
• Previous experience in home care setting with two years management or supervisory experience, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
