Job Juncture is seeking a full-time Patient Care Manager in Norwood, Massachusetts. In this role, you will provide clinical supervision to meet professional standards and ensure quality patient care.

Ideal candidates are registered nurses with the requisite licensure and have experience in home care settings, including management. You'll support staff growth through coaching and education.

Qualifications

• Registered nurse with current licensure to practice nursing in the practicing state.
• Previous experience in home care setting with two years management or supervisory experience, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements.

Job description

Schedule: Full-time | Monday – Friday | 8 a.m. to 5 p.m.

Key Responsibilities

• Provide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirements
• Coordinate and monitor patient care delivery
• Supervise clinical staff to ensure consistent, appropriate service
• Maintain clear communication with patients, caregivers, referral sources, and field and office staff
• Drive quality outcomes by meeting department goals and supporting improvement initiatives
• Model professionalism and organizational values in daily work
• Uphold compliance through required training, accurate reporting, and cooperation with audits
• Promote a safe work environment by following established safety protocols
• Support staff growth through coaching and continuing education
