Qualifications

• Registered nurse with current licensure to practice nursing in the practicing state.
• Two years management or supervisory experience in home health preferred.
• Knowledge of professional standards and Medicare Conditions of Participation and regulatory requirements.

Responsibilities

• Provide clinical supervision to ensure patient care aligns with standards, policies, and regulations.
• Coordinate patient care across the episode of care.
• Supervise clinical personnel to ensure services are delivered appropriately and on schedule.
• Communicate effectively with patients, caregivers, referral sources, and field and office staff.
• Drive quality outcomes by meeting departmental goals and contributing to quality improvement initiatives.

Patient Care Manager Qualifications

• Registered nurse with current licensure to practice nursing in the practicing state.
• Previous experience in home care setting with two years management or supervisory experience, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
