Responsibilities

• Evaluates learning needs of patient and family and provides education.
• Collaborates across disciplines to coordinate patient care.
• Delegates patient care to ancillary personnel.
• Precepts students and new hires.
• Plans, implements and evaluates care based on assessment to optimize outcomes.
• Monitors and documents patient condition and communicates updates.
