Job summary

Under provider direction, you will deliver office-based nursing care, assess patient needs, plan and implement care, and coordinate with physicians to ensure continuity of care.

The role requires a CT RN license and CPR certification; three years of practice preferred; travel to regional offices as needed.

Qualifications

• Provides skilled nursing care under provider direction.
• Assesses physical and psychosocial needs and plans nursing care.
• Triages telephone calls and initiates appropriate nursing interventions.
