Job summary

Jane Pauley Community Health Center in Indiana is seeking a Nurse to coordinate patient care and advocate across the care continuum. The RN will work with providers, staff, and external partners to ensure smooth transitions and chronic disease management.

This hybrid-remote role supports health promotion, disease prevention, patient education, and nursing care planning, with schedules tailored to the RN and JPCHC leadership, focusing on equitable access and quality outcomes.

Qualifications

• Current state licensure as an RN (required).
• Excellent communication and organizational skills.
• Strong critical thinking and decision-making abilities.

Responsibilities

• Advocate for patients and coordinate care across primary and specialty providers, hospitals, ERs, and JPCHC teams (care transitions).
• Provide patient education using evidence-based resources and promote self-care management.
• Support pre-visit planning and quality improvement initiatives; monitor care gaps and outcomes.
• Perform phone triage, medication refills, and prior authorizations within licensure.
• Document all patient interactions accurately in the EMR.
