Job summary

P3 Health Partners is seeking an RN Care Manager to ensure high-risk patients receive coordinated, continuous care in a hybrid in-office and telephonic setting.

You will use the nursing process to assess, plan, implement and evaluate care, develop and monitor care plans, collaborate with Medical Management, and promote self-management with patients and families. Licensure in NV/AZ/OR/CA and SNP/managed care experience are preferred.

Qualifications

• RN with current licensure in NV/AZ/OR/CA
• Case management certification or ability to obtain within two years
• CPR certification or ability to obtain within 60 days
• Graduate from an Accredited School of Nursing; Bachelor's preferred

Responsibilities

• Promote the mission, vision and values of P3 Health Partners
• Conduct comprehensive patient assessments via records, clinician and patient interviews, telephonic and face to face
• Develop, implement and evaluate patient care plans
• Collaborate with Medical Management on care plans and patient progress
• Identify care needs and barriers to care; develop plans to improve
• Prepare documentation per timelines and policy
• Coordinate care for high-risk patient population
• Support patients and families to promote self-management and meet goals
• Build relationships with PCPs and internal/external customers
• Address gaps in care
