Job summary

Cooper Norcross Health is seeking an experienced Outcomes Manager (RN) to lead outcome-based reporting for high‑risk patient groups and to drive performance improvement across hospital departments. Qualifications

• RN required with at least 5 years of clinical experience related to Core Measures or assigned patient populations.
• Experience with performance improvement concepts and CORE Measure indicators is preferred.
• Strong analytical skills to develop and monitor an outcomes-based reporting system.

Responsibilities

• Develop and monitor outcomes-based reporting for high-risk diagnoses and patient groups.
• Collaborate with interdisciplinary care teams to improve performance and regulatory reporting.
