Job summary

Cooper Norcross Health is seeking an Outcomes Manager (RN) with at least 5 years of clinical experience related to Core Measures to lead outcome-based reporting and performance improvement initiatives. This role collaborates with interdisciplinary teams to develop data-driven quality reports for regulatory bodies and hospital departments.

Requirements include RN licensure in New Jersey; BSN or MSN preferred and knowledge of quality control, Lean Six Sigma, and clinical pathways is advantageous.

Qualifications

  • 5 years recent clinical experience related to Core Measures or assigned patient population to oversee.
  • Prefer experience with performance improvement concepts and/or CORE Measure indicators.
  • RN required; BSN/MSN preferred.
  • Current New Jersey RN licensure required; RN certification preferred (CPHQ).

Responsibilities

  • Oversee data reporting for specific patient populations and hospital departments in collaboration with interdisciplinary teams.
  • Develop, implement, and monitor outcome-based reporting systems for high-risk diagnoses and care processes.
  • Communicate findings effectively to clinical staff and regulatory bodies; prepare quality reports as required.

Short Description
  • An Outcomes Manager is an RN with at least 5 years; clinical experience related to patient population(s) of specification.
  • Strong analytical skills necessary to develop, implement, and monitor an outcome-based reporting system for high-risk diagnoses, patient groups, physicians, and specific care processes.
  • Prefer experience with performance improvement.
  • Must possess effective communication skills, diplomacy and ability to work with all disciplines and levels.
  • Responsiblefor preparing quality reports related to the care of specific patient population or hospital clinical departments and for regulatory bodies in collaboration with members of the interdisciplinary care team.
  • Producesdata reports related to specific patient populations for hospital clinical departments and for regulatory bodies in collaboration with members of the interdisciplinary care team.
  • Required to have basic knowledge of quality control procedures and practices within a hospital environment.
  • Must have knowledge of data collection techniques, performance improvement methods and tools (e.g., Rapid Cycle PDSA, Lean Six Sigma, etc.).
  • Must have knowledge and understanding of clinical pathways or guidelines.
  • Knowledge of accreditation and regulatory requirements and standards is preferred.
  • Ability to communicate effectively orally and in writing, and the ability to work collaborativelywith others.
  • Ability to write reports containing technical information.
  • Ability to develop and present education programs and/or workshops; project management skills.
  • Team building and leadership skills. Ability to oversee the work of non-clinical dataabstractors, and to assist with their education regarding core measures data abstraction.
  • Time management and organizational skill with the ability to multitask unrelated activities.
  • Self motivated and able to work independently
Experience Required

Education Requirements

RN required. Bachelor of Science in Nursing (BSN) and/or Master of Science in Nursing (MSN) preferred.

License/Certification Requirements

Current New Jersey RN licensure required. Current RN certification preferred, especially CPHQ.
