Registered Nurse or Social Worker: Discharge Planning

Hours: Day shift 8:00am to 4:30am (No On-Call)
Where: Springfield MA

Responsibilities:

• Work with a few different departments to help with the discharge planning process.
• Speak with peers, patients and families during the discharge and follow them through the process.

Requirements:

• A Mass License in good standing.
• A few year of clinical experience.
• Worked with patients and families.
• Case management or U/R experience (this role is just DP).
