Job summary

Fort Worth Wellness and Rehabilitation Center is seeking a licensed LVN/RN to provide nursing care during the 10PM-6AM shift. You will work within scope of practice, follow physician orders, and adhere to center policies and regulatory guidelines.

Responsibilities include charting, incident reporting, narcotic counting, and coordinating patient care with physicians. Equal employment opportunities are provided to all applicants.

Qualifications

• Current LVN or RN license in Texas is required.
• BLS/CPR certification is required or willingness to obtain.
• Experience in nursing management is preferred but not required.

Responsibilities

• Follows established procedures for charting and incident reporting.
• Follows procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions including nutrition, infections, skin issues, weight loss, or other changes.
• Uses the Quality Life Manual to identify, place, and monitor patients in appropriate programs.
• Notifies appropriate departments of patient room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.

• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
