Job Description

Performs an age-specific plan of care for a designated group of residents using the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation of resident care. Collaborates with physicians and other team members in coordinating and implementing procedures and treatments. Uses leadership skills and clinical judgment in coordinating resident care and directing and delegating activities of the resident care team, specifically the LPN, CAN, and Unit Secretary.

Job Responsibilities

1. Utilizes the nursing process in providing care to assigned residents. Performs comprehensive physical and psychosocial assessments. Uses assessment data to determine residents' actual or potential problems. Plans appropriate nursing interventions and adjusts care as needed. Collaborates with physicians and team members to develop and implement care plans. Evaluates residents' responses to interventions. Administers prescribed medications accurately and timely, understanding their actions, indications, and potential adverse reactions. Participates in resident and family education, assessing the effectiveness of teaching. Engages in discharge planning activities, including initiating education for discharge. Documents assessments, care plans, and progress notes accurately, following departmental policies. Performs related duties as required.
2. Minimum Requirements

• Graduate from an accredited School of Nursing. Bachelor's Degree in Nursing preferred. Must be enrolled in an accredited program within 24 months of employment if hired after September 1, 2010, and obtain a BSN within five years of employment. Current license to practice as a Registered Nurse in New York State. BLS Certification. One year of experience, including charge nurse duties, Minimum Data Sets (MDS), and Patient Review Instruments (PRI).
