Job summary

Ambercare Hospice in Albuquerque, NM, is seeking a compassionate Weekend RN On-Call Nurse to provide expert clinical care to patients and support to families, ensuring comfort, dignity, and quality of life.

In this role you will manage a patient caseload, coordinate with physicians and the interdisciplinary team, perform visits, assess needs, and document care plans. A strong commitment to personalized care and professional growth is required, with full benefits offered.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation of the nursing process.
• Obtain data and assess information from patient and family factors that may impact needs and coordinate with the interdisciplinary team.
• Initiate communication with the attending physician and team for coordination of optimal care.
• Provide regular visits to re-assess patient needs and update the care plan.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records and care plans.
• Supervise and document delivery of care by Home Health Aide as required by regulations.

What You’ll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
